2. Improving the Coordination of Palliative Care

Santa Barbara is blessed with a number of organizations that provide excellent end of life care.  Within that system of care it is recognized that many of the areas for improvement lie within the interactions between agencies.  Often those problems stem from breakdown in communication or lack of integration of approaches and systems.

While many people feel the impact of those problems, and often recognize their source, finding solutions often rests outside of the control of any individual agency.  Consequently, sustained and supported collaborative efforts aimed at finding and addressing those issues are needed.  We seek to provide financial support for those efforts that will improve the coordination of care in Santa Barbara.
